The undersigned declares that they are aware of the provisions governing the reproduction of documents held in state public libraries and, in particular, of Legislative Decree No. 68 of 9 April 2003 and Articles 107–108 of the Code of Cultural Heritage and Landscape, and expressly declares, under their own responsibility, that the reproduced material will not be used for profit. False declarations are subject to the penalties provided for in Article 485 of the Criminal Code. (*)

The undersigned gives their consent to the processing of personal data in accordance with Legislative Decree No. 196 of 30 June 2003, Article 13. Such data will be processed exclusively by authorised personnel and will be transmitted only to the photographic laboratory responsible for supplying the requested reproductions. (*)
